Registration is now open for the 2018 Wellness Event. Reading through the vendor information and the registration form will answer most of your questions. Both the Registration Form AND Temporary Vendor Application need to completed. For questions on licenses please call Denise at 480-474-5070. Submitting an application for a booth doesn't guarantee space. All registration forms will be approved and a confirmation will be emailed.

* The $15 Temporary Vendor Application fee is not applicable IF a vendor is a non-profit agency, a hobbyist (profits less than $3,000 annually), or already possesses a valid City Business License
